Have your image file open in Photoshop.
Choose File>File Info...
You'll see a dialogue panel like the example below (in which the required information has already been entered).

Author: Ansel Adams
Description: (Where you put the large
caption) On October 21st, 2009, etc. etc
Copyright Notice: (Where
you put the small caption) Humpback Whale washed up on Town Beach in
October 2009. etc.
